Comprehensive Medication Review CMR

Did you know that poor medication reconciliation is a leading cause of polypharmacy in older adults? And actually, adults in general!

The dangers of poor medication reconciliation are stockpiles of expired drugs or testing supplies, duplicated drugs, look-alike tablets, sound-alike names. These can lead to poor adherence, accidental overdose, increased side effects and even hospitalization.

One way to avoid these possible dangers is to have a Comprehensive Clinical Medication Review (CMR) that evaluates all the medications, disease states, OTCs and more. A CMR can open the lines of communication between provider(s) and patients and lead to Deprescribing, better adherence and safety, resulting in a better quality of life for the patient.
